What is Public Provident Fund (PPF) scheme ?
PPF scheme is central Government backed scheme framed under PPF Act, 1968 to provide retirement security to self employed individuals , workmen in unorganized sector.

List of Public Provident Fund (PPF) Account Opening Forms :
Form A – Public Provident Account Opening Form
Form A is PPF account opening form which you need to fill in and submit along with required documents to the bank you want to open PPF account.
Form B – To make deposit into loan taken against a PPF account
This is a deposit form used to deposit repayment amount in loan taken against PPF account. Amount can be deposited via cash, Cheque, DD etc.
Form C – Partial Withdrawal form from PPF account
You can also withdraw partial amount from the account from year 7 of opening of PPF account. Form C is used to withdraw certain sum from PPF account. You need to fill in PPF account number, amount to withdraw in form along with declaration form stating that you have not withdrawn any amount during the same financial year.
Form D – Loan against PPF account request form
You need to request in Form D to avail loan facility against amount held in PPF account from year 3 to year 6 of opening of the account.
Form E – Addition of Nominee in a PPF account
Fill Form E is for addition of Nominee in PPF account. More than one nominee can be nominated for a single account. Fill all the details related to nominees, their address , relation along with percentage of funds that can be claimed by nominee.
Form F – Change of Nomination information in PPF account
You can also change the details of nominee or cancel the nomination. You can change or cancel the nominee at any point of time during the tenure.
Form G – Death Claim Form by Legal Heirs / Nominee
After the PPF account holder deceased, legal heirs or nominee can claim the amount held in PPF account by filling the Form G enclosed with death certificate of account holder.
Form H – Extension of PPF account Form
As you know tenure for PPF account is 15 years but it can be extended further by 5 years. Account holder need to submit the Form H to extend the tenure.

How to fill PPF account opening form A :
It is very simple to fill PPF account opening form-A.
- Write your full name, Paste a recent passport size photograph on form.
- Fill the PAN card number correctly.
- Mention whether you want to open PPF account in your name or in name of your minor child of whom you are guardian.
- Fill the initial deposit / subscription amount.. by Cash or Cheque.
- Read the declaration given on the account opening form.
- Fill the details of other PPF account if you are maintaining in the name of minor / HUF or in name of association of persons.
